MEDICARE EPC GUIDE

You may be eligible if your GP has documented a chronic condition, such as diabetes or heart disease, that has been or is likely to be present for six months or longer.

Your GP will assess your condition and, if appropriate, can write a referral under the EPC Plan. This referral allows you to access up to five subsidised sessions per year with allied health professionals.

Any chronic condition that requires ongoing care from multiple health professionals, such as arthritis, asthma, cancer, cardiovascular disease, diabetes, and mental health conditions.

You can have up to five sessions per calendar year subsidised by Medicare under the EPC Plan. Each session includes any of the eligible services provided by our clinic.

Yes, you can continue treatment; however, Medicare will not cover the costs after the five sessions. Our staff can provide you with information about the costs and payment options. Check out our pricing and Book Online Here.

The rebate is available for services provided by eligible allied health professionals, such as Physiotherapists, Chiropractors, and Dietitians, as long as these services are included in your GP’s care plan.

Bring your Medicare card, referral form from your GP, and any relevant medical reports or tests. Also, wear appropriate clothing for assessment and treatment.
